How do I get a copy of the adoption order in New York?

After the finalization hearing, the Family Court or Surrogate's Court produces the signed Order of Adoption. Here is how to get copies:

At the hearing:

• The judge signs the Order of Adoption during the hearing
• Ask the court clerk for certified copies right after the hearing
• Some courts provide copies the same day; others mail them within 1–2 weeks

How many copies to request:

• We recommend 3–5 certified copies — you will need them for:

- New York State Department of Health Vital Records (to request new birth certificate)

- Health insurance enrollment (qualifying life event)

- School records update

- Passport application

- Your personal records

Cost per certified copy: Typically $5–$25 per copy

If you need copies later:

• Contact the clerk of the Family Court or Surrogate's Court where the adoption was finalized
• Provide the case number and date of finalization
• Pay the copying/certification fee
• Copies are usually available within a few business days

What to do with the adoption order:
