Florida's adoption amendment fee is $20 and includes one certification of the amended birth record. The currently linked DH429 application also lists $1 shipping and handling for U.S. postal delivery, making its basic mailed application total $21 before optional copies or rush service.
Use the Florida Department of Health's adoption-amendment instructions and the DH429 application currently linked there. These charges concern a Florida record. If the birth was recorded elsewhere, that jurisdiction's requirements and prices apply.
Work out the number of copies you need
The following examples use the current DH429 mail-application charges: $20 amendment including one certificate, $9 for the first additional certificate, $4 for each further certificate, and $1 U.S. postal shipping and handling.
| Certificates requested together | Worked mail-application total, without rush |
|---|---|
| One | $20 + $1 = $21 |
| Two | $20 + $9 + $1 = $30 |
| Three | $20 + $9 + $4 + $1 = $34 |
Confirm the current form and chosen delivery method before paying. The form separately lists an optional $10 rush fee and says expedited shipping is not included. Do not assume that paying for rush changes the court's reporting time or supplies a faster shipping service.
Check which office holds the birth record
The Florida Department of Health says the clerk forwards an adoption report within 30 days of the order. That report is used to establish the new Florida record. Ask whether the clerk has forwarded it and whether the records office has received the information needed to complete your request.
If the person was adopted in Florida but born in another state, Florida forwards the adoption report to the birth state. Contact that state's vital-records office for its application, fee and eligibility requirements. Filing an adoption in Florida does not make another state's certificate a Florida certificate.
Foreign birth records involve a separate set of questions. A Florida certificate of foreign birth is not proof of U.S. citizenship. For that issue, use the reviewed citizenship guide and the relevant official immigration or passport instructions.
Follow the application's eligibility instructions
DH429 identifies eligible applicants, requires a completed application and valid photo identification, and explains evidence of eligibility. Use the official instructions for the person requesting the record, including an adult adoptee where applicable. An adoption order does not mean that anyone who knows the family can order its confidential birth record.
Check the new name, the name on the existing record and the court information before applying. Use the official document and appropriate records office; this guide is a planning aid, not an application or a submission service.
Plan timing without promising a delivery date
The current form gives a general amendment-processing estimate of two to three weeks depending on workload. It also says a certificate cannot issue until the required evidence, forms, fees and signatures meet the applicable criteria. That estimate is not an all-in deadline beginning on adoption day.
Keep a copy of your application, payment details and any records-office correspondence. For a Florida adoption-amendment question, DH429 lists (904) 359-6900, extension 9001. Confirm current contact instructions before calling.
Avoid ordering copies on an assumed rule
Ask the school, insurer or other recipient whether it needs an original certification, will return it, or accepts another document. Choose a quantity based on those answers instead of assuming every family must order three to five copies.
A court's certified adoption order and a vital-records birth certificate are different documents with different charges. Keep both on your Florida adoption budget, or use the adult-adoption cost guide for an adult case. Ask the clerk about order copies separately from the records office's certificate charge.
